So I gather that this sort of terrain modification can not be done via the default TS functions?

I actually prefer the 'pyramid' style look to the terrain in this route. It's basically like recreating an old 1990s game so that it can run on more modern operating systems. Don't forget that this route is from the early 2000s, so it's not exactly modern TS payware standards.

But nonetheless, if I was to make a 3d modeled hill in TS, what shader would I use so that it would mix with the terrain visually?

How about just setting the terrain tool on the far left to some large number and just pull it up?

I just noticed that switching from circular to square shape hills makes a slight difference. Nonetheless, it will still be an arduous task to attempt to replicate this terrain with reasonable accuracy.

Is there any way at all to copy MSTS terrain data into some kind of format that TS will recognise for importing?
