That may be your problem right there.
You extract the exe, but you don't extract the "Railworks" folder with the actual repaint files in it.
Your setup exe is probably installing whatever old files it finds on your desktop (check if you already have a "Railworks" folder on your desktop). You may have a lot of old stuff on your desktop that you should clean up.
Here's what to do:
1- Download the zip file from RWA. Save it in any temporary folder you want (like "My documents/RWA").
2- Open the zip file and extract all of its contents into the same temporary folder. You should now have both the setup exe and the "Railworks" folder inside this temporary folder, along with the zip file which you don't need anymore, the readme file, one or two jpeg files, and maybe other files.
3- Right-click on the setup exe. Choose "Run as administrator".
4- The installer will display the path to your Railworks installation. Check this and confirm.
5- The installer will now copy the files from the "Railworks" folder inside the temporary folder, to your railworks installation.
6- Ignore any Windows messages about the program maybe not installed successfully.
7- Check the readme for any additional steps, such as manually copying a geopcdx file.
8- Delete the temporary folder.
